Care coordination, also some times called "targeted case management", is a system of communication and coordination between the client and the service providers capable of supporting them. Care coordinators are like a mediator between a client and their services, and while they don't directly provide care services themselves, they are a crucial step in getting the assistance you may need. The process to become a client of a care coordinator starts with an application with ADRC (Aging and Disability Resource Center). ADRC connects seniors, people with disabilities, and care givers with long-term care services and support systems of their choice. One of these supports is care coordination. ADRC will guide you to the best possible avenue of care through their systems.
After ADRC has evaluated your case, you may be determined to be eligible for " Home and Community Based Waiver programs". If you're uncertain what Home and Community Based Waiver programs are, as many people may be, we'll go a bit more in depth. From the State of Alaska SDS Website: "Waiver programs offer people who meet an institutional level of care the opportunity receive care in the home or community instead of in an institution." What this means is that you can be integrated into community based care facilities, or that you can receive certain services in your own home. The goal of the Alaska waiver program is to help intermingle those who experience life differently than others into the community wherever possible.
This is when certified care coordinators step into the picture. A certified and skilled care coordinator has the experience and resources to connect you with the right service providers based on your needs and level of care required. Not every one will need to be placed into an 'assisted living facility' (A home that is outfitted to assist those who are differently able, such as having wheelchair accessibility, or that have 24hr nursing assistance). Care coordinators have the experience to help determine what would best serve your needs. This is accomplished by open communication with you as a client to discover your needs, wants, and goals. After all, this is about you as a person and we want your voice to be heard. The care coordinator then uses this information to help create plans (support plans) with service providers. These plans help you meet and hopefully even exceed your goals while maintaining a healthy and happy lifestyle!

Jakson Kitsune is Oni's right hand in all office operations. Jakson plays a pivotal role in the day to day operations of 9 Tales Care Coordination LLC. Jakson brings a wealth of experience in the IT field and dedication to improve existing systems. As office manager, Jakson wears many hats and performs a diverse range of tasks to support Oni and 9 Tales Care Coordination LLC.
